Forming Good Habits for Life

Star Valley Medical Center’s diabetes education is available to all people with diabetes or impaired glucose tolerance along with their family members. Individual instruction is provided by certified healthcare professionals. Appointments can be made by calling 307-885-5932 Monday through Friday from 8:30 a.m. to 4 p.m. Please contact your physician to coordinate services.

Individual Instruction

Managing diabetes requires the formation of good daily habits. Patients enrolled in our program will receive the following instruction to help them along their way:

Management skills for people with all types of diabetes and related issues
Oral medications
Insulin therapy
Beginning and updating self-monitoring of blood-glucose
Group training sessions
Intensive diabetes management
Insulin pump therapy
Establishing and/or updating individual meal plans
Carbohydrate counting
Long-term complications
Hemoglobin A1c testing
Foot screenings
Exercise programs
Sick day management
